My advice would be to first contact the Life in the UK test helpline on 0800 015 4245, just to reassure yourself of the procedure – but having spoken to them myself, this is what they told me:
When you’re online on the Home Office website filling in your citizenship application, you will be asked for your unique reference number for your test.
You won’t have a reference number, and so you put a zero in the box.
It will then ask for supporting evidence – you will then need to write a covering letter saying to the best of your knowledge when and where you took the Life in the UK test.
The good news is that the Home Office does have a record of who’s passed the Life in the UK test and so if you give them the best information you can, they should be able to find you.
